You will be responsible for assisting in the provision of specialist palliative care and end-of-life care to older persons, residents/ participants, families and other stakeholders who are facing palliative care end of life care needs.Key areas of Responsibilities:Maintain a person-centred approach in assessment, planning, delivery and evaluation of the older person’s care and service provisionDeliver clinical support tailored to the older person’s individual needs and take a leading role in the development of palliative care and end-of-life care plansProvide continuity of care to ensure effective palliative care and or end-of-life careIdentify opportunities for the development of palliative care and end-of-life care nursing services to improve the care of the older personManage therapeutic interventions and referrals as required together with strong palliative care and end of life care pharmacology knowledgeIn consultation with residents, participants, families, CCCs and GPs, coordinate review and the revision of care plans for palliative care and end-of-life carePromote collaborative working practices in all aspects of palliative care and end-of-life careDeliver In-Service Education Program, Toolbox Talks to all relevant personnel, residents, participants and familiesSupport and provide education and knowledge transfer about palliative care and /or end-of-life care to residents / participants, their families and staffParticipate in case conferences and monthly care statements for residents / participants in regard to palliative care and /or end-of-life careMonitor and evaluate care provided and provide ongoing assessment of resident / participant health status in accordance with defined policy and procedures.The Successful Candidate will have the following experience and skills:Dedication to resident care and supportAt least five years’ experience as a Registered Nurse (Div 1)Strong knowledge and experience working with Palliative care and end of life care technology and educationDeveloping palliative care and end-of-life care plansAbility to interpret aged care and disability standards and develop and / or implement related policies and proceduresExcellent interpersonal, written and verbal communication skills Ability to effectively communicate with residents, resident representatives, and other health professionalsA good understanding of the aged care reform agenda, including AN-ACC funding and royal commission recommendations related to clinical care.We offer a competitive salary, rewarding career opportunities and pride ourselves in providing a work environment that is safe, flexible, and promotes a healthy work life balance.  AUD Footscray 3011
